By 'Actively' adding your name to national Organ Donor Register, you're making your intention clear to everyone involved.

All you need to do is fill out the form below with your information and preferences.

Once you’ve registered you’ll be sent a thank you email from YES I DONATE. You'll also be sent a letter and an Organ Donor card to confirm that you're now an Organ Donor, along with all your details to check.

Once you've registered you just need to GET YOUR T-SHIRT. Get it, wear it and share it.
